Job summary

Personal Healthcare Management LLC is currently seeking Licensed Practical Nurses for an overnight shift in Town of Delhi, NY. This full-time role offers 12-hour night shifts from 7:00pm to 7:00am, three days a week, with every-other weekend.

Compensation starts at $35 per hour, with evening and night differentials up to around $36.75/hr, and a sign-on bonus of $5,000 over two years. A comprehensive benefits package and professional growth opportunities are provided.

Qualifications

• Licensed Practical Nurse (LPN) with current license.
• Minimum 1 year of experience as LPN preferred.
• Willingness to work 12-hour night shifts and overnight schedule.

Responsibilities

• Provides safe resident care and completes treatments per standards.
• Administers medications according to five rights and documents accordingly.
• Participates in quality improvement processes and shift handoffs.
• Communicates care plans and updates during handoffs and rounds.

Functions

• Provides safe resident care, demonstrating knowledge and competency in completing assigned treatments and procedures according to established standards and policies. Maintains awareness of new updates standards and incorporates them into his/her nursing practice throughout the continuum of care.
• Participates in the improvement process of quality resident care.
• Incorporates age-specific needs, provision of care, procedures, and treatments.
• Communicates pertinent resident information and plan of care before breaks, during transfer, and at shift report.
• Accurately administers medications according to the five rights of medication administration and documentation of same.
• Accurately performs treatments utilizing sterile or clean technique.
• Accurately administers external feedings.
• Completes resident care-related activities within scheduled shift time periods.
• Cooperates with reassignments made in the interest of resident care.
• Reports resident care problems and possible solutions to the appropriate nursing management personnel.
