Job summary

Personal Healthcare Management LLC seeks LPNs for the Overnight Shift in Fraser, NY. This full-time role features a 12-hour night schedule from 7:00pm to 7:00am, three days a week and every other weekend, with a $5,000 sign-on bonus over two years.

Starting wage is $35.00 per hour, up to $35.50 per hour, with Eve and Night differentials. Benefits include health, dental, vision, 401(k), and paid time off, along with growth and stability opportunities.

Qualifications

• 1 year nursing experience
• Licensed Practical Nurse (LPN) license required
• Overnight shift availability
• Night shift 7:00pm–7:00am with every other weekend

Responsibilities

• Provides safe resident care, following standards and policies.
• Maintains updated knowledge and applies new practice standards.
• Considers age-specific needs in care, procedures, and treatments.
• Communicates resident information and plan of care during shifts and handoffs.
• Administers medications according to the five rights and documents accurately.
• Performs treatments using sterile or clean technique.
• Administers external feedings as required.
• Completes care activities within scheduled shift times.
• Cooperates with reassignments for resident care.
• Reports problems and solutions to nursing management.
• Delegates care tasks appropriately.

Functions

• Provides safe resident care, demonstrating knowledge and competency in completing assigned treatments and procedures according to established standards and policies. Maintains awareness of new updates standards and incorporates them into his/her nursing practice through out the continuum of care.
• Participates in the improvement process of quality resident care.
• Incorporates age-specific needs, provision of care, procedures, and treatments.
• Communicates pertinent resident information and plan of care before breaks, during transfer, and at shift report.
• Accurately administers medications according to the five rights of medication administration and documentation of same.
• Accurately performs treatments utilizing sterile or clean technique.
• Accurately administers external feedings.
• Completes resident care-related activities within scheduled shift time periods.
• Cooperates with reassignments made in the interest of resident care.
• Reports resident care problems and possible solutions to the appropriate nursing management personnel.
• Delegates resident care problems and possible solutions to the appropriate nursing management personnel.
