Avenir Behavioral Health Center is seeking an Intake Coordinator for weekend shifts (Saturday Day & Night) to support admissions and referrals. You will coordinate referrals, manage bed availability, and build patient charts within the EMR to ensure accurate initial documentation.

Responsibilities

• Referral & Facility Coordination: manage incoming referrals, contact referring agencies, and communicate bed availability.
• Admissions Scheduling & Charting: coordinate admissions and create clinical charts in the EMR for arriving patients.
• Assessments & Clinical Consents: complete intake assessments and obtain admission consents.
• Legal & Guardian Documentation: obtain required admission paperwork, including guardian consents when applicable.
• Patient Asset Management: log patient belongings and store items securely.
• Data Integrity & Tracking: maintain intake logs, admission trackers, and facility logs across shifts.
• Accountability & Productivity Tracking: maintain daily shift productivity logs.
• Operational Flexibility: perform other duties as assigned by leadership.
