Job summary

Second Chances Addiction Recovery Center seeks an Intake Coordinator to manage daytime client intakes and ensure accurate documentation and orientation. The role supports staff when intake sessions are not occurring and enhances operations beyond regular hours.

You will handle initial screenings, complete intake for admissions, and maintain EMR accuracy while coordinating with nursing and admin teams to ensure smooth client transitions.

Qualifications

• High school diploma or equivalent required.
• Associate/Bachelor’s in Human Services, Psychology, Social Work, or related field preferred.
• 2+ years of admissions experience in behavioral health, substance abuse, or medical settings preferred.

Responsibilities

• Serve as the first point of contact for daytime client inquiries.
• Conduct initial screenings and gather necessary documentation.
• Complete intake for clients admitted on shift, ensuring paperwork and EHR documentation is accurate.
• Audit intake charts for completeness, compliance, and accuracy.
• Provide orientation and support to clients and families during the admission process.
• Communicate with on-call or supervisory staff regarding new admissions, discharges, or urgent issues.
• Enter intake documentation into the EMR system promptly.
• Ensure intake charts comply with facility, state, and federal regulations (LDH, Medicaid, CARF, HIPAA).
• Maintain up-to-date records in the EMR system.
• Assist with administrative duties such as scanning, filing, or data entry.
• Provide support to residential staff/techs with client needs as assigned.
• Coordinate with nursing, residential staff, and on-call administrators for smooth transitions.
• Maintain professional communication with referral sources, clients, and staff.
