Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to deliver compassionate, patient-centered care in patients’ homes. You will assess needs, provide nursing care per the full nursing process, and work closely with the Interdisciplinary Group (IDG) to develop individualized plans of care.

Responsibilities include assessments, planning and updating the Plan of Care, administering medications, on-call duties, and detailed documentation.

Qualifications

• Associate or Bachelor’s degree from an accredited School of Nursing.
• 1 year experience practicing as a registered nurse.
• Current, unencumbered RN license.
• National Provider BLS - American Heart Association within 30 days of hire.

Responsibilities

• Completes assessment of patient and family to determine Hospice needs.
• Writes and initiates Plan of Care with the IDG.
• Regularly reevaluates patient/family needs and revises the Plan of Care.
• Develops a care plan with patient/family/IDG based on nursing diagnosis.
• May administer medications and treatments as prescribed by the IDG Plan of Care.
• Participates in on-call duties as defined by on-call procedures.
• Maintains productivity standards and documents clinical notes.
• Supervises ancillary personnel and contributes to team education.
