Job summary

Reserve Care is seeking an On-Call Registered Nurse or Licensed Practical Nurse to provide after-hours nursing care in patient homes around Cleveland, OH. This role emphasizes compassionate support, symptom management, and caregiver guidance in urgent evening to early morning hours.

Responsibilities include assessing needs, administering intermittent skilled nursing, communicating with physicians, and coordinating referrals.

Qualifications

• High School diploma or equivalent required.
• Graduate of an accredited nursing program; BSN preferred.
• Licensed RN/LPN in Ohio.
• Experience in pain/symptom management, oncology, or mental health preferred.
• Excellent communication and the ability to work both independently and with a team.

Responsibilities

• Assess patient and family needs (RN).
• Gather data on patient and family needs (LPN).
• Provide intermittent skilled nursing services under physician direction.
• Monitor and manage pain and symptoms; support patient and family.
• Act as liaison with physician; relay information as needed.
• Provide emotional support to patient and family.
• Refer to other services when appropriate.
• Instruct patients and families on home care.
