Qualifications

• Graduate of an accredited school of nursing.
• Current licensure in Montana.
• 1–2 years of recent acute care experience; 1–2 years in home care preferred.

Responsibilities

• Completes an initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Develops and revises the plan of care with the interdisciplinary team.
• Administers medications and treatments as prescribed; counsels patient and family on care needs.
• Acts as Case Manager for assigned caseload and coordinates patient care.
