Job summary

Stillwater Hospice, LLC in Helena, MT is seeking a Hospice Registered Nurse Case Manager to plan, direct and coordinate home- and community-based hospice care. You will assess patient/family needs, educate and support them, and work with an interdisciplinary team to implement tailored care plans.

Qualifications

• Holds a nursing license valid for the state where work occurs.
• Experience in acute care and home care settings is preferred.
• Strong communication and patient/family education skills.

Responsibilities
