Job summary

Qualifications

• Graduate of an accredited nursing program with current state licensure.
• 1–2 years of acute care experience and 1–2 years in home care preferred.
• Valid driver's license with reliable vehicle required.
• Bachelor’s degree in Nursing preferred.

Responsibilities

• Performs initial and ongoing patient assessments to determine hospice needs.
• Develops and revises care plans with patient and family involvement.
• Administers medications and treatments as prescribed by the physician.
• Coordinates with interdisciplinary team to ensure quality care.
• Participates in on-call duties and manages equipment needs.
