Job summary

Stillwater Hospice, Helena, MT, is seeking a dedicated Registered Nurse Case Manager to lead patient care in home settings. The role demands clinical expertise and strong communication with families and the care team.

As a Hospice RN, you will assess patients, develop plans of care, coordinate services, and provide education and support. On-call duties and a competitive hourly rate accompany comprehensive benefits.

Qualifications

• Graduate of an accredited school of nursing.
• Current licensure as an RN in the state.
• 1–2 years acute care experience and 1–2 years in home care preferred.

Responsibilities

• Performs comprehensive patient and family assessments and develops care plans.
• Delivers professional nursing care per the plan of care and state Act requirements.
• Coordinates patient care with the hospice interdisciplinary group.
• Provides patient and family education and support for home-based hospice care.
• Participates in on-call duties and ensures equipment and services are arranged as needed.
