Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse to deliver compassionate, patient-centered care in patients’ homes. You will assess needs, develop plans with the IDG, and provide nursing care through the full nursing process to support end-of-life comfort and dignity.

The role emphasizes community health, interdisciplinary collaboration, and ongoing education of team members. Part-time, day shift with Sat/Sun 10-hour shifts; emphasis on high-quality hospice care.

Qualifications

• Required - Associate or Bachelor's degree from an accredited School of Nursing.
• Required – Current, unencumbered RN license (State of Employment).
• Within 30 days of hire - National Provider BLS - American Heart Association (Vendor Managed).
• Preferred - 1 year experience practicing as a registered nurse. Experience as a Hospice CNA or Hospice LPN will be considered.

Responsibilities

• Completes assessment of patient and family to determine Hospice needs.
• Provides a complete physical assessment and history and provides professional nursing care by utilizing all elements of the nursing process.
• Writing and initiating Plan of Care in collaboration with the IDG.
• Regularly reevaluating patient/family/caregiver needs and revising the Plan of Care as necessary.
• Initiates the Plan of Care and makes necessary revisions as patient status and needs change.
• Develops a care plan in conjunction with the patient/family/caregiver and IDG which establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
• May administer medications and treatments as prescribed by the IDG approved Plan of Care.
• Participates in on-call duties as defined by the on-call procedures.
• Maintains productivity standards as set by organization guidelines.
