Job summary

Providence at Home with Compassus seeks a compassionate Hospice Registered Nurse (RN) to deliver patient-centered care in patients' homes. You will assess needs, provide professional nursing care, and collaborate with the Interdisciplinary Group (IDG) to develop individualized care plans that honor dignity and comfort at end of life.

The role emphasizes community health education, care planning, and on-call duties while maintaining high clinical standards.

Qualifications

• Associate or Bachelor’s degree in Nursing from an accredited school.
• Preferred 1 year of RN experience in hospice or applicable setting.
• Current RN license in good standing.
• National Provider BLS certification within 30 days of hire.

Responsibilities

• Assess patient and family needs and determine Hospice requirements.
• Provide comprehensive nursing care following the nursing process.
• Write and initiate Plan of Care in collaboration with the IDG.
• Regularly reevaluate needs and revise the Plan of Care as needed.
• Administer medications per the Plan of Care and participate in on-call duties.
• Maintain accurate clinical notes and participate in team education.
