Providence at Home with Compassus is seeking a dedicated Hospice Registered Nurse (RN) to deliver compassionate, patient-centered care in home settings. You will assess needs, provide professional nursing care, and collaborate with the Interdisciplinary Group to plan and direct individualized care plans for patients and families during end-of-life care.

The role emphasizes community health, hospice nursing expertise, and educating families.

Qualifikationen

• Required - Current, unencumbered RN license in the state of employment.
• Required within 30 days of hire - BLS from American Heart Association.
• Preferred - 1 year of hospice RN experience; hospice CNA/LPN experience considered.

Aufgaben

• Completes assessment of patient and family to determine Hospice needs.
• Provides a complete physical assessment and history and professional nursing care per the nursing process.
• Writes and initiates Plan of Care in collaboration with the IDG.
• Regularly reevaluates patient/family/caregiver needs and revises the Plan of Care as necessary.
• Initiates the Plan of Care and makes revisions as patient status changes.
• Develops a care plan with patient/family/caregiver and IDG based on nursing diagnosis and palliative actions.
• May administer medications and treatments per the IDG Plan of Care.
• Participates in on-call duties as defined by on-call procedures.
• Maintains productivity standards per organizational guidelines.
