Job summary

Providence at Home with Compassus is seeking a registered nurse to plan, implement, and oversee skilled home health services in collaboration with the interdisciplinary team. You will deliver care in patients’ residences, coordinate with physicians, families, and referral sources, and ensure compliance with care plans.

The role covers assessment, case management, plan of care development, infection control, on-call coverage, and thorough documentation.

Qualifications

• Graduate of an accredited college/university, Associate, Diploma, or Baccalaureate School of nursing.
• Current, unencumbered State License as a registered Nurse
• Current Driver’s License
• Current CPR, negative TB screen and Hepatitis consent/declination

Responsibilities

• Assess home health patients to identify physical, psychosocial, and environmental needs from records and team input.
• Case manages and provides clinical oversight to the interdisciplinary team, physician, and family.
• Implements and documents the plan of care and revises as necessary.
• Provide care with infection control measures protecting staff and patient (OSHA).
• Supervise and direct HHA/LVN to ensure quality and continuity of services.
• Ensure continuity of quality patient care with proper documentation.
• Monitor cases for compliance with third party payor requirements.
• Perform on-call duties and provide on-call service as assigned.
• Communicate effectively with patient/family, team, physicians and other health professionals.
• Adhere to company policies and information systems security policies.
