Job summary

Providence at Home with Compassus is seeking a Hospice Registered Nurse (RN) to deliver compassionate, patient-centered care in home settings. You will assess patients and families, administer nursing care and collaborate with the Interdisciplinary Group to develop and direct individualized plans of care.

Responsibilities include completing assessments, writing and revising the Plan of Care, providing medications as prescribed, participating in on-call duties, and maintaining accurate

Qualifications

• Associate or Bachelor’s degree in Nursing from an accredited program.
• Current, unencumbered RN license in state of employment.
• Hospice or home-health experience preferred.

Responsibilities

• Complete patient/family assessments to determine hospice needs.
• Provide professional nursing care using all elements of the nursing process.
• Write and initiate the Plan of Care with the IDG and update as needed.
• Reevaluate needs and revise the Plan of Care as patient status changes.
• Administer medications as prescribed and participate in on‑call duties.
• Maintain accurate clinical notes and participate in team education.
