Responsibilities

• Deliver patient‑centered care in hospice settings with an interdisciplinary team
• Perform comprehensive assessments, symptom and pain management, and end‑of‑life care planning
• Educate families and support them through the end‑of‑life journey
• Maintain compliant EMR documentation and travel to patient homes within territory
