Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) to join our Hospice team serving Whitney, Pennsylvania. This after-hours role focuses on compassionate end-of-life care delivered in patients' homes, with a 40-hour week and 8-hour shifts.

You will collaborate with an interdisciplinary team, manage symptoms, and provide support to patients and families during evenings, nights, weekends, and crisis situations. A valid RN license and driver’s license are required.

Qualifications

• Active RN license in state you work is required.
• Two years of RN experience with hospice experience preferred.
• A valid driver's license is required.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team.
• Respond to after-hours and on-call patient needs by performing urgent assessments and managing symptoms.
• Support and guide patients and families during evenings, nights, weekends, and crisis situations.
• Apply clinical judgment to coordinate care and maintain EMR documentation.
• Work independently, but in a team-based environment.
• Travel to patient homes within assigned territory.
