Job summary

Graham Healthcare Group is hiring a Hospice Registered Nurse to provide compassionate end-of-life care to patients in their homes across the Mars, PA area. This full-time role requires an active RN license and hospice experience, with about 20% travel to patient locations.

You'll work with an interdisciplinary team, document thoroughly in EMR, and support families through difficult times. A $10,000 first-year bonus is offered, in addition to our comprehensive benefits package.

Qualifications

Active RN license in the state you work.
Two years of RN experience with hospice preferred.
A valid driver’s license and reliable transportation.

Responsibilities

Deliver compassionate, patient-centered hospice care in patients' homes with an interdisciplinary team.
Perform comprehensive assessments, symptom and end-of-life care planning.
Educate and support patients and families throughout the end-of-life journey.
Maintain thorough, compliant EMR documentation.
Work independently, but collaborate with the team and travel to patient homes.
