Job summary

Graham Healthcare Group is seeking a Registered Nurse (RN) to provide skilled home health care in Muse, Pennsylvania. The position requires weekend availability, 40 hours per week, and a consistent 8-hour shift.

You’ll assess patients, manage medications, educate families, and document care in the EMR. A PA RN license, valid driver’s license, and at least one year of home health experience are required; support for growth is provided.

Qualifications

Active RN license in the state you work.
One year of RN experience in home health or similar.
Valid driver’s license and ability to spend approximately 20% of your workday driving to and from patient locations.

Responsibilities

Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
Perform patient assessments, medication management, and care planning.
Educate and empower patients and families on disease management and treatment plans so they feel confident, informed, and cared for.
Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation.
Work independently, but never alone.
Travel to patient homes within assigned territory.
