Job summary

Lehigh Valley Health Network, Inc. is seeking a Weekend Hospice Visit Registered Nurse (RN) to provide compassionate home hospice care.

The role is part-time, with weekend shifts (Saturday/Sunday/Monday; 8:00am - 8:30pm) and a $20,000 sign-on bonus plus a $5,000 honor for National Home Care and Hospice Month. You will collaborate with a multidisciplinary team and support patient/family goals in the Lehigh Valley area.

Qualifications

• Current RN license in Pennsylvania
• BLS certification within 30 days of hire
• Experience delivering home health or hospice care preferred
• Strong communication and time-management skills
• Able to travel to patient care assignments

Responsibilities

• Deliver patient care per physician-approved plan and protocols
• Perform ongoing assessments and report changes to the care team
• Educate patients and families on end-of-life care and disease management
• Coordinate with RN Case Manager and interdisciplinary team
• Document care activities and patient status in the EMR
