Lehigh Valley Health Network is seeking a Registered Nurse for Hospice Admissions on weekend days in Allentown. You will determine hospice eligibility, perform comprehensive assessments, and coordinate a patient-centered plan of care with the interdisciplinary team.

The role emphasizes education for patients and families, symptom management, and end-of-life support. The position requires a BSN or ADN with hospice experience and an active RN license in Pennsylvania.

Qualifications

• Bachelor’s Degree in Nursing or Associate’s Degree in Nursing with 1 year hospice experience
• 1–2 years RN experience in hospice or acute care settings
• Ability to travel within defined service area and work with interdisciplinary team

Responsabilités

• Perform initial and follow-up hospice assessments per CMS requirements
• Develop individualized care plans based on physician orders and data
• Educate patients and families on end-of-life care and goals of care
• Communicate changes in patient status promptly to the care team
• Deliver direct nursing care with emphasis on end-of-life comfort and symptom management
