Job summary

Lehigh Valley Health Network, Inc. is seeking a Registered Nurse Hospice Case Manager to coordinate and direct patient care in the home setting. The role requires collaboration with an interdisciplinary team, developing and updating care plans, and delivering skilled interventions in accordance with physician orders.

Qualifications

• Active RN license in Pennsylvania
• 1 year of direct patient care experience
• Knowledge of CMS Conditions of Participation
• Ability to travel to patient care assignments
• Excellent time management and organizational skills
• Strong verbal and written communication skills
• Basic Life Support (BLS) certification

Responsibilities

• Coordinate and direct home-based care for an assigned caseload with the interdisciplinary team.
• Develop, initiate, and revise the care plan in collaboration with the physician.
• Deliver skilled treatments and patient/caregiver education.
• Ensure regulatory data collection with high accuracy for CMS and reimbursement.
• Promote education on end-of-life, palliative care, and discharge planning.
