Job summary

Lehigh Valley Health Network in Allentown is seeking a Hospice Visit Nurse - Registered Nurse (RN) for weekend shifts to deliver patient-centered care under the medical plan and in collaboration with the interdisciplinary team.

This role requires a NLN-approved nursing program, active RN license in PA/NJ, BLS, and strong communication; a $15,000 sign-on bonus is offered. You will assess patients, educate families, and coordinate discharge planning to enhance comfort and quality of life.

Qualifications

• NLN approved/accredited nursing program.
• 1 year recent direct healthcare experience.
• Knowledge of patient education techniques and principles.
• Ability to work independently within the care team.
• Ability to travel to patient care assignments.
• Knowledge of Conditions of Participation.
• Strong verbal and non-verbal communication.
• Excellent time management and organization.
• BLS and AHA within 30 days; PA RN license upon hire.
• Driver's license for PA/NJ upon hire.

Responsibilities

• Deliver patient care per medical plan and protocols.
• Perform ongoing assessments of patient status.
• Relays changes to physician and care team in a timely manner.
• Revise care plans with RN Case Manager input.
• Provide education on end-of-life, palliative care, and discharge planning.
• Support patient/caregiver autonomy and comfort.
