Job summary

Lehigh Valley Health Network, Inc. is seeking a Weekend Hospice Admissions Registered Nurse to support hospice admissions on weekend days in Allentown, PA.

You will determine hospice appropriateness, perform assessments, and develop a patient-centered care plan with the interdisciplinary team. The role emphasizes end-of-life care, family education, and collaboration with physicians to ensure quality outcomes.

Qualifications

• Bachelor’s or Associate degree in Nursing with hospice/home health experience.
• Current RN license for PA and BLS certification.
• Ability to travel within the defined service area to patient care assignments.

Responsibilities

• Assess patients to determine hospice needs and level of care per CMS requirements.
• Perform initial and follow-up hospice assessments and formulate an individualized care plan.
• Collaborate with the interdisciplinary team and educate patients and families on end-of-life care.
