Job summary

Lehigh Valley Health Network is seeking a Weekend Shift Registered Nurse for Home Care Admissions to assess patients, determine care levels, and develop individualized plans in collaboration with the interdisciplinary team.

The role requires an NLN-approved nursing program and at least 1 year of direct patient care, with strong communication skills and the ability to travel to patient care assignments. A $10,000 sign-on bonus plus $5,000 extra if you apply by Labor Day is offered.

Qualifications

Specialized nursing program with 1 year of direct care experience
Knowledge of patient education techniques and principles
Ability to work independently and travel to patient care assignments
Familiarity with CMS regulations and data collection
Effective verbal and non-verbal communication skills

Responsibilities

Perform initial and follow-up assessments for home care patients
Determine level of care and plan of care with interdisciplinary team
Educate patients and families on care plans, end-of-life care, and discharge planning
Collaborate with physicians and care team to adjust care as needed
Relay changes in patient status promptly to the medical team
