Lehigh Valley Health Network is seeking an Admissions Nurse - Registered Nurse (RN) for Home Care and Hospice on the weekend shift. You will determine appropriate levels of care, perform initial and follow-up assessments, and develop individualized care plans with the physician and interdisciplinary team.

The role emphasizes patient-family education and coordinated discharge planning. Minimum qualifications include an NLN-approved nursing program and 1 year of direct care experience.

Qualifications

• Specialized Diploma NLN approved/accredited nursing program; 1 year recent experience providing direct care in a healthcare setting.
• Knowledge of patient education techniques and principles
• Ability to take initiative and function independently within a care team
• Knowledge of Conditions of Participation
• Knowledge of and skill in completing regulatory data set collection with accuracy
• Ability to facilitate difficult and complex communication related to goals of care
• Exhibits excellent time management and organization skills
• RN license PA/NJ upon hire and driver's license as applicable

Responsabilités

• Performs comprehensive subjective and objective initial and follow-up assessments of patient status including physical, psychological and environmental parameters.
• Completes data collection per CMS regulations with high accuracy reflecting quality outcomes measures.
• Formulates an individualized plan of care according to physician orders and interdisciplinary collaboration.
• Relays significant changes in patient status to the physician and care team promptly.
• Delivers patient care based on the medical plan of treatment established by physician and team.
• Provides patient, family, and caregiver education on end-of-life, palliative care, chronic disease management, and discharge planning.
• Promotes patient/caregiver autonomy and supports the care team in decision-making.
• Collaborates with physician and interdisciplinary team to determine appropriate level of care and skilled services.
• Ensures compliance with regulatory requirements and participates in ongoing quality improvement efforts
