Job summary

Lehigh Valley Health Network (LVHN) is recruiting a Weekend Hospice Visit Registered Nurse to deliver patient-centered care in the home setting, with a Friday/Saturday/Sunday schedule from 8:00 am to 8:30 pm. The position includes a $25,000 sign-on bonus and a potential extra $5,000 for National Home Care and Hospice Month, contingent on application timing.

The role emphasizes end-of-life care, patient and family education, and collaboration with the interdisciplinary team and RN Case Manager.

Qualifications

• Nursing program approved by NLN or equivalent.
• 1 year recent direct care experience in healthcare.
• Ability to travel to patient care assignments.

Responsibilities

• Delivers patient care per physician and interdisciplinary plan using a family-centered approach.
• Performs ongoing assessments and communicates changes to the care team.
• Revises care plan in collaboration with RN Case Manager as needed.
• Educates patients and families on end-of-life care and discharge planning.
