Job summary

Affinity Health Management in Florida seeks a dedicated registered nurse to lead hospice care in patients' homes and communities, emphasizing education, assessment, care planning, and coordination with the interdisciplinary team.

You will perform comprehensive assessments, develop and adjust care plans, administer treatments, educate families, and serve as case manager for assigned caseload. Candidates must be licensed and have recent hospice or home health experience.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse, with a License in good standing to practice.
• RN experience within last 3 years or BSN with 2 years’ experience, at least one in public health/home hospice.
• Management experience not required; responsible for supervising hospice aides.
• Offer contingent upon references and criminal background checks.
• Excellent observation, verbal and written communication skills; nursing skills per competency checklist.
• Must be able to lift, stand/walk for long periods; drive with insured vehicle.

Responsibilities

• Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care by utilizing all elements of nursing process.
• Writes and initiates plan of care; revises as necessary.
• Initiates the plan of care and revises as patient status changes.
• Uses health data to determine nursing diagnosis.
• Develops a care plan with goals, including patient and family in planning.
• Initiates appropriate preventive and rehabilitative nursing procedures; administers medications as prescribed.
• Counsels patient and family in meeting nursing and related needs.
• Provides health care instructions per assessment and plan.
• Acts as Case Manager for assigned caseload and coordinates patient care.
• Participates in on-call duties and quality improvement activities.
