Job summary

Affinity Hospice Management LLC seeks a registered nurse skilled in hospice care to plan, direct, and coordinate services in patients' homes and communities across Florida.

The role emphasizes comprehensive assessments, care planning, family education, and collaboration with interdisciplinary teams to deliver quality palliative care. On-call duty and weekend availability are expected.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in Florida.
• 1+ year nursing experience in home health/hospice preferred.
• Driving license and insured vehicle required.
• CPR certification desired.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Provides nursing care using the nursing process and plans of care.
• Writes and initiates plan of care; revises as patient status changes.
• Administers medications and treatments as prescribed by physician.
• Counsels patient and family on nursing needs and care instructions.
• Assists with activities of daily living and supports self-care and comfort.
• Acts as Case Manager for assigned caseload when required.
• Maintains accurate clinical notes and communicates with physicians and team.
