Job summary

Affinity Health Management in Florida is seeking a Registered Nurse with home care or hospice experience to lead patient care in home settings. The RN will assess patients, develop and implement care plans, administer medications, and educate families.

The role requires Florida RN license, strong communication, and ability to work in community-based hospice, with on-call duties and weekend coverage.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse license in good standing to practice in Florida.
• 1+ year of professional nursing experience within the last 3 years, or BSN with at least 2 years’ experience (preferably in public health/home/hospice).
• Excellent observation, verbal and written communication skills; problem solving; basic math.
• Must have valid driver's license and reliable insured vehicle; CPR certification required.

Responsibilities

• Completes initial, comprehensive and ongoing patient/family assessment to determine hospice needs.
• Writes and initiates plan of care and revises as needed.
• Administers medications and treatments as prescribed by physician.
• Maintains clinical notes and communicates with physician and care team.
• Acts as Case Manager for assigned caseload; coordinates patient care.
• Provides patient and family education and home care instructions.
• Participates in on-call duties and weekend coverage.
• Maintains safe environment and supports continuity of care.
