Job summary

Affinity Health Management seeks a Registered Nurse with hospice/home health experience to plan, direct, and deliver care in patients' homes and communities in Florida. The RN will develop and supervise care plans, provide patient/family education, and coordinate interdisciplinary team activities.

Responsibilities include comprehensive assessments, plan of care development, medication administration, and ongoing coordination with physicians.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing to practice in Florida.
• Minimum of 1 year of professional nursing experience within the last 3 years, or a bachelor’s degree with at least 2 years’ experience (preferably in public health, home care, or hospice).
• Excellent observation, communication, problem-solving, and math skills.

Responsibilities

• Completes initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
• Delivers professional nursing care using the nursing process; initiates and revises the care plan as patient needs change.
• Maintains and submits accurate clinical notes; communicates with physicians and care team regarding patient status.
• Provides patient and family education, assists with activities of daily living, and acts as Case Manager for assigned caseload.
