Job summary

Shalom Hospice in Knoxville, TN is seeking a Registered Nurse to plan, organize, and direct hospice care in patients' homes and communities. The role emphasizes community health education and collaboration with families to meet patient needs.

The RN will assess patients, develop and revise care plans, administer treatments, and coordinate with the care team to ensure quality care and comfort at home.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with a license in good standing to practice.
• Minimum one year of nursing experience within the last three years or a BSN with two years in public health/home care/hospice preferred.
• Management experience not required; can supervise hospice aides.
• Offer contingent on references and criminal background checks.
• Excellent observation, verbal and written communication skills, problem solving, and math skills; nursing skills per competency checklist.
• Must be able to lift/transfer patients and have a valid driver's license with insured vehicle.

Responsibilities

• Perform initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient and family involvement.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family in nursing and related needs; provide health instructions.
• Coordinate patient care as case manager when assigned; liaise with physician and interdisciplinary team.
