Peoples-Home-Health, LLC is seeking a Hospice Admissions Nurse to perform hospice consultations and admission visits, assess patient and family needs, and collaborate with physicians to determine hospice eligibility. The role includes completing admission documentation, coordinating care, and supporting the patient and family through the transition to hospice services.

You will manage a caseload, participate in on-call rotation, and ensure timely communication with the IDG and case managers.

Qualifications

• Current Florida or multi-state RN license is required.
• Two years of RN experience preferred.
• CPR certification is required.
• Valid driver’s license and auto insurance required.

Responsabilités

• Perform hospice consultations and admission visits to assess patient and family needs.
• Collaborate with attending physicians to determine hospice eligibility.
• Develop initial Plan of Care (POC) with IDG and patient/family.
• Coordinate medications and DME for same-day delivery.
• Maintain accurate records and complete admission documentation on time.
