Job summary

Reserve Care (formerly Hospice of the Western Reserve) is seeking a Clinical Admissions Nurse to ensure admissions for patients in home, nursing facilities, or other care settings. You will collaborate with the Referral Department and physicians to promote hospice care and ensure timely certifications.

The role requires nursing expertise, compassionate patient interaction, and adherence to HIPAA policies. Weekend schedule is Saturday and Sunday, 9:00 AM–9:30 PM, with 24 scheduled hours and

Qualifications

• Graduate of an accredited nursing program with BSN and/or CHPN.
• OH RN license required.
• Experience: intake, home care, hospice and/or oncology nursing preferred.
• Drivers license and insured vehicle as required.

Responsibilities

• Assures admissions and information sharing to patients and families.
• Coordinates with Referral Department and physicians to ensure appropriate referrals.
• Provides nursing care, education and emotional support to patients and families.
• Maintains documentation in electronic medical records within agency timelines.
