Job summary

Carle Health is seeking a Home Care RN/Field Case Manager to coordinate and deliver skilled nursing in patients' homes across central Illinois. The role involves independent work within an interdisciplinary team of clinicians and aides, performing assessments, care planning, and education for patients and families.

Qualifications include IL RN license, BLS within 30 days, and a nursing diploma with at least 1 year of nursing experience.

Qualifications

• IL RN certification required with BLS within 30 days.
• College diploma in nursing and valid driver license.
• Minimum 1 year nursing experience is required.

Responsibilities

• Coordinate and provide direct/indirect patient care using the nursing process.
• Perform complex wound care including wound vacs and dressings.
• Monitor central lines (PICC/Hickman) and blood draws per protocol.
• Complete admission, transfer, and discharge documentation timely.
• Communicate with MDs, care coordinators, and families to plan care.
