Job summary

UnitedHealth Group is seeking a Physical Therapist (PT) to deliver in-home therapeutic services, aiming to improve mobility, balance, and safety for patients in their homes. You will collaborate with a multidisciplinary team to support recovery and reduce hospitalizations, with a schedule of Sunday through Thursday for full-time hours.

The ideal candidate holds a CAPTE-accredited PT degree, current state license, CPR certification, and at least 1 year of home health experience.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• 1+ year of home health PT experience preferred; mentoring may be required for less experience.
• Current CPR certification.

Responsibilities

• Provide skilled physical therapy services in patients' homes to promote mobility and independence.
• Perform reexaminations at least every 30 days and develop discharge plans as needed.
• Collaborate with patients, caregivers, and an interdisciplinary care team to support recovery.
