Job summary

Support Solutions in Bolivar, TN is seeking a Direct Support Professional for weekend nights (Saturday and Sunday, 7 PM–7 AM). This part-time role provides hands-on assistance to individuals with disabilities, helping them with daily living, personal care, and community participation.

The job emphasizes safety, documentation, and teamwork, with weekly pay, a $300 sign-on bonus after 90 days, and a $300 referral bonus for successful referrals.

Qualifications

• Must be 18 years of age and able to read and write effectively.
• ECF staff must have high school diploma or GED.
• Must have a valid driver’s license and maintain a valid driver’s license and acceptable driving record.
• Must meet state and local requirements regarding criminal background check, sexual offender check, abuse registry, drug and felony offender listings.
• Must meet all training requirements and keep training current.

Responsibilities

• Supports the person supported with personal care and daily routines, and in achieving outcomes from the ISP.
• Helps the person supported become integrated with the community by providing person centered supports.
• Assures safe transport and accompanies the person supported as needed to events, job sites, and medical appointments utilizing agency vehicles, personal vehicles, or public transportation.
• Documents daily shift notes and uses electronic records system as required by funding entities.
• Ensures customer service standards when interacting with persons supported, their families, conservators, vendors, ISCs, Case Managers, and others.
