Qualifications

• Ability to read and comprehend simple instructions, short correspondence, and memos.
• Ability to write simple correspondence.
• Ability to present information in one-on-one and small group settings.
• Basic math and data interpretation skills.
• Computer skills: data entry and basic software navigation.
• Reasoning to follow written or oral instructions.
• Physical ability to stand, walk, lift up to 100 pounds, and perform tasks.

Responsibilities

• Assist clients with basic living skills and daily tasks including showers, dressing, housekeeping, meals, and budgeting.
• Provide transportation to activities and appointments; valid driver’s license required.
• Maintain communication via logbooks and staff meetings on client needs and progress.
• Complete required documentation such as incident reports, care sheets, and progress reports.
• Follow Medicaid guidelines for vehicle use, mileage, and billing forms.
• Report incidents or emergencies immediately; safety-sensitive role.
• Support in care plan development and monitoring for safety and treatment adherence.
• Follow medication management guidelines and document procedures.
• Organize and supervise recreational activities for clients.
