Responsibilities

• Assist clients with basic living skills and daily tasks (showering, dressing, housekeeping, meal planning).
• Provide transportation to activities and appointments as needed.
• Maintain documentation such as Behavioral Reports, Incident Reports, and Progress Reports.
• Support Plan of Care implementation and monitor client health and safety.
• Follow HIPAA policies and confidentially maintain client information.
• Model professional behavior and maintain facility cleanliness and vehicle maintenance.
