Compassus seeks a Weekend Float Clinical Care Partner to coordinate safe, patient-centered transitions of care for hospitalized patients in Nashville. The role emphasizes discharge planning and collaboration with physicians, case management, and post-acute providers to ensure appropriate home-based care services and timely handoffs.

Required RN, LCSW, or LMSW licensure, with a Full-time Weekend Float schedule covering Friday to Monday shifts, including 4/10 or 4/8 hour blocks.

Qualifications

• Experience in care coordination, discharge planning, or related healthcare services preferred.
• Experience with EMR systems (e.g., Epic) and referral platforms preferred.
• Ability to educate patients and families about post-acute options and care pathways.

Responsibilities

• Coordinate safe, efficient transitions of care for hospitalized patients.
• Evaluate patients for home-based and post-acute services.
• Collaborate with physicians, case management, and post-acute providers.
• Lead discharge planning conversations and create individualized transition plans.
• Arrange post-acute services including home health, hospice, and medications.
• Support value-based care initiatives and accurate documentation.
