Qualifications

• Graduate of an accredited school of nursing with RN license.
• Minimum two years of acute hospital clinical experience.
• Masters in Case Management or Nursing preferred.

Responsabilités

• Completes and documents discharge planning assessments for identified patients.
• Facilitates multidisciplinary discharge planning with care teams and post-acute providers.
• Oversees and evaluates discharge plan implementation.
• Collaborates to ensure timely transitions and appropriate inpatient resource use.
• Identify readmission risks and implement mitigation strategies for 30 days.
