Job summary

Ascension at Home together with Compassus is seeking a Weekend Care Transition Associate to provide non-clinical administrative and care coordination support. You will assist with referrals, discharge planning, and coordinating services for home health and hospice needs.

This role requires weekend coverage and remote work within the Eastern Time Zone. Ideal candidates have an Associate’s degree or equivalent experience, plus at least one year of patient-facing service in healthcare.

Qualifications

• Associate’s degree or equivalent education and experience.
• A high school diploma or equivalent plus three years of work experience is equivalent to an Associate’s degree.
• One year of experience working with patients and families and providing customer service and administrative support.
• Two or more years of healthcare experience and related coursework are preferred; proficiency with EMR systems.

Responsibilities

• Serve as a non-clinical liaison between inpatient and outpatient settings for referrals and discharge coordination.
• Coordinate referral and admission activities and collect/verify patient information for starts of care.
• Support clinical assessment by organizing information on patient appropriateness.
• Collaborate with clinical teams to coordinate services and equipment per orders and needs.
• Communicate with physicians and care teams to verify orders and address discharge barriers.
• Provide administrative support including EMR documentation, chart organization, and information distribution.
