Qualifications

• Associate's degree is required.
• Bachelor's degree is preferred.
• 2–3 years of healthcare experience is preferred.
• No prior liaison/discharge planning experience is mandatory.

Responsibilities

• Coordinate discharge planning and transitions to post-acute care.
• Evaluate patients for appropriate home-based and post-acute services.
• Educate patients and families about post-acute options and services.
• Loster with physicians, case management, and nursing teams to align care.
• Document referrals and care plans in the EMR and track transitions.
