Ascension at Home together with Compassus is seeking a Clinical Care Partner to coordinate safe, patient-centered transitions of care for hospitalized patients and evaluate post-acute home-based care options. This in-person role requires bedside engagement and collaboration with physicians, case management, and nursing teams.

The position emphasizes discharge coordination, documentation, and education for patients and families, with a focus on reducing readmissions and ensuring appropriate

Formación

• Active and unencumbered RN, LMSW, LCSW, or LICSW licensure.
• Current CPR certification.
• Associate’s degree in Nursing, Health Sciences, or related field; or equivalent healthcare experience.

Responsabilidades

• Coordinate safe, patient-centered transitions of care for hospitalized patients.
• Evaluate patients for home-based post-acute care and discharge planning.
• Collaborate with physicians and care teams to decide level of care.
• Arrange post-acute services including home health, hospice, and DME.
• Provide bedside education to patients and families about post-acute options.
• Support discharge planning workflows and communication between hospital and post-acute providers.
