Job summary

Ascension at Home together with Compassus seeks a Clinical Care Partner III to coordinate safe, efficient transitions of care for hospitalized patients in Indianapolis, IN. The role focuses on discharge planning, post-acute services, and collaboration with physicians and care teams.

This in-person position requires bedside engagement, interdisciplinary teamwork, and adherence to value-based care and care coordination workflows.

Qualifications

• Education: An accredited nursing degree (RN) or related clinical licensure; Master of Social Work (LCSW) or Master’s in Speech Language Pathology.
• Experience: Preferred 2–3 years in care coordination, discharge planning, or post-acute care.
• Certifications: RN/LCSW/Speech Therapist licensure as applicable.

Responsibilities

• Coordinate discharge planning and transitions to post-acute home-based care.
• Review referrals, prioritize patients, and support level-of-care decisions.
• Arrange post-acute services including home health, hospice, and equipment.
