Job summary

Continuum of Care is seeking a program-focused professional to provide comprehensive planning and active supports for clients, while coordinating with community providers. The role includes developing service plans, supervising direct care staff, and ensuring regulatory compliance.

The position requires a Bachelor’s in behavioral health or equivalent, with 1+ year of related experience, valid CT driver’s license, and CPR/First Aid certification or willingness to obtain within 90 days.

Qualifications

• Bachelor’s degree in behavioral health or related field and minimum experience in behavioral health.
• High School or equivalent with 3+ years in related healthcare field is acceptable.

Responsibilities

• Provide case management supports to assigned caseload.
• Assist in supervision and training of direct care staff, interns, and volunteers.
• Support program management in monitoring costs and allocating resources.
• Conduct on-call coverage and crisis intervention as clinically indicated.
• Maintain and document client records and communications with community providers.
