Job summary

Connecticut Community Care, Inc. seeks a Care Manager to assist individuals in informed decision-making about Long-Term Services and Supports, coordinating formal and informal caregivers, and delivering person-centered care that honors the consumer’s choices.

You will conduct comprehensive assessments, educate clients on program options and eligibility, complete documentation, monitor care plans, and collaborate with an interdisciplinary team, including on-call coverage.

Qualifications

• Education: Bachelor’s degree in administration, social work, public health, recreation, psychology, counseling, gerontology, or related field required.
• Experience: Minimum of two years’ experience in health care or human services; substitution allowed with related degrees.

Responsibilities

• Educates individuals on the components of the program, service options, and DSS guidelines, including eligibility, costs, and benefits.
• Promptly completes all client documentation, applications, forms, and additional documentation as required.
• Monitors and reviews care plans, service delivery, renewals, and contractual obligations; revises plans with the individual as needed.
• Conducts person-centered telephone and in-person interviews with clients and families for reassessment and monitoring.
• Works as part of an interdisciplinary team and participates in on-call services; mentors new staff as needed.
