Job summary

Qualifications

  • Bachelor’s degree in administration, social work, public health, recreation, psychology, counseling, gerontology, or related field.
  • Minimum of two years’ direct case management experience.
  • Bilingual preferred.
  • Knowledge of community resources; Medicaid/Medicare helpful.
  • Familiarity with funding sources and service options.
  • Experience conducting comprehensive assessments in various settings.
  • Strong advocacy and communication skills.
  • Reliable transportation and valid driver’s license.

Responsibilities

  • Addresses individuals’ needs through comprehensive, person-centered assessments in preferred settings.
  • Educates individuals on program components, options, eligibility, costs, and benefits.
  • Completes client documentation, applications and required forms promptly.
  • Monitors care plan quality, updates service orders and renewals with providers.
  • Conducts interviews with clients and families for reassessment and plan adjustment.
  • Collaborates with interdisciplinary team and participates in on-call coverage.
  • May mentor new staff and participate in continuing education.
