Job summary

Continuum of Care in New Haven, CT seeks a Direct Support Professional to assist individuals with intellectual disabilities. The role emphasizes respectful, dignified care, daily living activities, and community involvement, aligned with IPs and behavioral programming.

You’ll support medical, dental, and personal appointments and help access community resources. Requirements include a diploma or degree preference, valid CT driver’s license, CPR/First Aid, and basic computer skills for

Qualifications

• Bachelor’s or Associate’s degree preferred; high school diploma with related experience considered.
• Valid Connecticut driver’s license with a clean driving record.
• CPR & First Aid certification required or preferred; PMT and DDS/CoC certifications as applicable.
• Computer skills to navigate agency electronic records and document care activities.

Responsibilities

• Provide face-to-face, hands-on support for activities of daily living and personal care.
• Assist individuals to live independently in their own home and participate in community activities.
• Document daily IP teaching, ADL strategies, behavioral and medical needs in designated systems.
• Input data in Therap or other electronic systems and report as required.
• Attend and participate in staff or designated meetings and perform other tasks as assigned.
