Job summary

Continuum of Care in New Haven, CT is seeking a Case Manager II to support Housing First initiatives. This full-time position covers community-based case management, screenings, assessments, and person-centered care planning.

You will collaborate in a team, coordinate referrals, access entitlements, and assist clients with daily living skills, transportation, and community resources. A valid driver’s license is required and CPR/First Aid is offered.

Qualifications

• Bachelor’s degree with 1+ year experience; or a high school diploma with 3 years’ experience.
• Experience with people with persistent chronic mental illness and/or addiction.
• Valid driver's license and reliable vehicle in community settings.
• CPR/First Aid certification offered and maintained.
• Proficient reading, writing and oral skills; computer literacy.
• Knowledge of community resources preferred.

Responsibilities

• Establishes rapport and builds positive client relationships while maintaining boundaries.
• Develops and monitors person-centered care plans from intake to discharge.
• Coordinates referrals, entitlements, treatment access, housing and transportation.
• Supports clients in daily living skills and transportation needs.
• Maintains accurate electronic records and program documentation.
• Participates in meetings and safety drills as required.
